To request registration forms, or to arrange for a tour of the school, please contact the school office at (630) 231-1776. Please use the checklists below to guide you in completing the paperwork necessary to register students for enrollment in St. Mary's School. Students registering for kindergarten must be five years old by September 1st. |
Registration Checklist for New Families
Registration for new families will be accepted when all
of the following items are complete.

Complete and return the Student Information Sheet.

Copy of the child’s Birth Certificate.

Copy of the child’s Baptism Certificate.

Health Records for beginning Preschool, Kindergarten, 5th and/or new students from outside the State of Illinois. Physicals must include physician’s authentic signature and be on the new state form, which has two sides. Inoculations now include chicken pox.

A non-refundable registration fee.

One non-refundable tuition deposit per family. This deposit retains classroom positions for your children.

Tuition Payment Option Agreement form.
Registration Checklist for Current Families

Complete and return the Student Information Sheet.

Health Records for students beginning Preschool, Kindergarten and 5th grade.

All health examination forms must include the physician’s authentic signature and be on the current state form.
State Mandated Medical Physicals: Completed forms turned in by October 1st for Kindergarten and 6th grade students
State Mandated Dental Examinations: Completed forms turned in by May 1st of 2010 for Kindergarten, 2nd, and 6th grade students.
State Mandated Eye Examinations: Completed forms turned in by October 1st for Kindergarten students.
Sports Physicals: Required annually for 5th-8th grade students who participate in sports.
