New Families
Registration for new families will be accepted when
all of the following items are complete.

Complete and return the Student Information
Sheet |
|
|
Copy of the child’s Birth Certificate
|
|
|
Copy of the child’s Baptism Certificate
|
|
|
Health Records for beginning Preschool,
Kindergarten, 5th and/or new students from
outside the State of Illinois. Physicals
must include physician’s authentic signature
and be on the new state form, which has two
sides. Inoculations now include chicken pox.

|
A non-refundable registration fee of $25.00
|
|
|
One non-refundable tuition deposit of
$250.00 per family with children in
Preschool-8th or one non-refundable tuition
deposit of $150.00 per family with children
in Preschool only. This deposit retains
classroom positions for your children.
